There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Care Co-Ordinator Duties Action GP referrals ensuring correct processes are followed to avoid unnecessary delays for patients Review all incoming correspondence filing the document and entering pertinent information into the patients electronical medical record in accordance with the practice protocol Support patients with Long-Term Conditions (diabetes, asthma, COPD etc) to arrange annual reviews ensuring follow-up appointments and tests are scheduled as appropriate Identifying high risk patients using various searches and ensuring appropriate tests and reviews are completed as necessary Support national vaccination campaigns (flu, RSV, shingles, pneumovax etc) by identifying and inviting eligible patients Maintain high uptake in childhood immunisations by supporting the child health team ensuring patients are invited and followed up as necessary Support national screening programmes (bowel screening, cervical screening, AAA screening etc) by identifying and inviting eligible patients when required Maintain accurate records ensuring all appropriate data is recorded for QOF, QIF etc Provide assistance and chaperoning to GPs when required Recognise own knowledge limits and seek advice from clinical or administrative colleagues as appropriate To carry out administrative related tasks as delegated by GP Partners or Practice Manager Inputting and extracting data as required to support practice reports Reception Duties Be the first point of contact and effectively greet patients and visitors to the surgery in an appropriate and professional manner at all times Book, cancel and rearrange appointments as requested by telephone, face-to-face or online using the electronic appointment book, ensuring patients are directed to the most appropriate healthcare professional Communicate effectively with patients, relatives and carers Process in person, by telephone and e-mail general enquiries ensuring accuracy of detail in all messages and ensuring these are actioned as necessary Action all practice tasks and workflow comments daily in accordance with the practice policy Explain practice procedures and arrangements for new patients, Registration of patients residing within the practice boundary, ensuring appropriate documents are completed and information entered onto system is correct Provide results of tests and investigations as requested and advised by the GP Update patient details to ensure correct and accurate details are held, i.e. correct address and telephone number, and monitor any PDS differences Handle and redirect internal and external mail appropriately and accurately scan and attach clinical and administrative information/correspondence onto patients records as necessary Issue correct and appropriate specimen containers and forms and ensure the correct information is obtained and completed on return of the specimen Raise service requests with members of the PCN, wider healthcare community and other services
