We are seeking a dedicated and compassionate Care Coordinator to join our team. The Care Coordinator will play a vital role in ensuring that patients receive the appropriate care and support throughout their healthcare journey. This position requires excellent organisational skills and the ability to work effectively in an office environment, liaising with various healthcare professionals and patients to facilitate seamless care.
Duties
Coordinate patient care by scheduling appointments, managing follow-ups, and ensuring that all necessary documentation is completed accurately.
Serve as the primary point of contact for patients, addressing inquiries and providing information regarding services and treatment options.
Collaborate with healthcare providers to develop individualised care plans tailored to each patient's needs.
Maintain comprehensive patient records, ensuring confidentiality and compliance with all relevant regulations.
Monitor patient progress and communicate updates to relevant stakeholders, including family members and healthcare teams.
Assist in the development of educational materials for patients to enhance their understanding of their health conditions and treatment plans.
Participate in team meetings to discuss patient cases and contribute to continuous improvement initiatives within the organisation.
Requirements
Proven experience in an office environment, preferably within a healthcare setting.
Strong organisational skills with the ability to manage multiple tasks simultaneously while maintaining attention to detail.
Excellent communication skills, both verbal and written, with a compassionate approach towards patient interactions.
Proficiency in using office software applications (e.g., Microsoft Office Suite) and electronic health record systems.
Ability to work collaboratively as part of a multidisciplinary team while also being self-motivated.
A background in healthcare or related fields is advantageous but not essential.
