Job Description

We are looking for a practice nurse with a special interest in diabetes focuses particularly on caring for patients with diabetes, running diabetes clinics and long-term condition reviews within the practice, Approx 16-20hrs per week, can be flexible with hours.

Typical Duties and Responsibilities

Run clinics for long-term conditions (e.g., diabetes, asthma, COPD) and deliver routine health checks. Take blood/urine samples, and monitor blood pressure and pulses. Administer vaccinations and immunisations (childhood and adult). Give advice on health promotion, contraception, blood pressure, smoking cessation, weight management, etc. Provide wound care, dressings and minor treatments. Support patients in managing long-term conditions and lifestyle changes.
Diabetes-Focused Duties

Practice nurses with a diabetes speciality:

Run diabetes care and review clinics for patients with Type 1 or Type 2 diabetes. Monitor blood glucose control, HbA1c, blood pressure and other indicators. Initiate or adjust insulin and other treatments according to protocols (sometimes with prescribing rights). Educate and support patients and carers on self-management, diet, lifestyle, devices (like glucose monitoring). Work with GPs and the wider team to refer to specialists or secondary care when needed.
They may also be the first point of contact for diabetic patients within the surgery.

Qualifications and Skills Required

Registered Nurse (RGN/RMN) with the Nursing & Midwifery Council (NMC). This is essential. Experience and interest in primary care/general practice is very helpful. Additional training or qualifications in diabetes management required. Good communication, clinical assessment skills, and ability to work within protocols and multidisciplinary teams
