Location:
Exeter / Hybrid Working (Mix of remote, prison, and community).Contract:
Full-Time, Part-Time, or Flexible Working (Monday-Friday).Closing Date:

.Role Purpose:
To ensure aseamless transition
for individuals released from custody.The Role: Intensive Transition Support
You will provide intensive, person-centred support for both men and women, focusing on recovery and reintegration.
Key Responsibilities
Manage a personal
caseload
. Provide12 weeks pre-release
engagement and12 weeks post-release
support. Complete strength-basedassessments
and deliverharm reduction interventions
. Coordinaterelease day logistics
(transport and support). Build and maintain effectivepartnerships
with Housing, Probation, and Drug & Alcohol providers.Essential Experience & Skills
This role requires
compassion, resilience, and specific knowledge of:
Working within the
Criminal Justice System
(Prisons, Courts, Probation). Supporting individuals affected bysubstance use and recovery
. Experience inmulti-agency/partnership environments
. Strongcaseload management, assessment, and communication skills
