About the Role
This is a rewarding opportunity to join our dedicated hospice social work team. We work closely with patients, families, and carers to support them with the emotional, social, and practical impact of life-limiting illness.
You'll be part of a multidisciplinary team, working alongside our inpatient and community nursing teams, as well as a range of internal and external services. Your role will mainly involve supporting people in their own homes, though there will some sessions at our new community hubs and occasional work on our inpatient unit.
You'll manage your own caseload with the support of our experienced team. The ability to work independently and with sensitivity is essential.

Band 4 salary, 27,061 - 29,696 FTE, dependent on experience
30 hours per week, Monday - Thursday.
(If up for discussion, 4 week days that must include Mondays and Wednesdays)

About You
We're looking for someone who:
Brings compassion and emotional intelligence to their work
Has experience in social care (experience in palliative or long-term conditions is an advantage but not essential)
Holds a full driving licence and has access to a car (our services cover Bristol, South Gloucestershire and North Somerset)
