The Maples Community is a group of organisations working together to provide specialist rehabilitation, recovery and accommodation services in the southwest.
We specialise in providing community based solutions to people with complex unmet needs that are transitioning out of psychiatric hospital, or are at risk of readmission.
We have divisions dedicated to working with people who have been diagnosed with complex post-traumatic stress disorder, learning difficulties, enduring acute mental health disorders or have forensic histories.
Summary
Deliver an innovative and compassionate clinical service?working with our frontline staff to?take a hands-on person-centred approach in identifying opportunities to deliver care packages
Deliver a recovery focused?clinical offering using a wide range of bespoke care planning?and risk management strategies.
Oversee the delivery, action and review of Clinical Care/Support?Plans?using recovery focused strategies ensuring person centred care
Assess risk and contribute to the development?of robust management strategies in line with positive risk frameworks and guidance
Lead group sessions and 121 sessions as required
Work in partnership with?The Parks MDT to deliver a recovery focused?clinical and therapeutic service
In partnership with the Clinical lead to?oversee the delivery, action and review of Clinical Care/Support?Plans?using recovery focused strategies ensuring person centred care
Work collaboratively with service users, carers, internal and external stakeholders and take an active role with cross agency working
Able to assess risk and contribute to the development?of robust management strategies in line with positive risk frameworks and guidance
Understand and abide?by all policies, procedures, legislation and guidance related to clinical practice
Ensure accurate, robust and complete documentation of all records
Develop and maintain knowledge of mental health illnesses, sections and strategies?for rehabilitative outcomes
Actively participate in?multi-agency?working?i.e. Safeguarding, MCA, DOLS?processes ensuring service users are central to the process and?measures implemented are least restrictive
Conduct crisis?mitigation and resolution work
Ensure relevant information?is effectively?disseminated throughout the internal MDT and wider stakeholders
Participate in internal and external MDT meetings
Support Senior Management Team in policy formulation, review and implementation within the service
Maintain knowledge of policies, procedures, legislation and guidelines in relation to clinical practice
Liaise?with Registered Manager and Clinical lead on all clinical related issues
Provide ongoing peer support within your team and provide guidance and leadership to junior staff members
Ensure effective working relationships are maintained
Foster strong stakeholder engagement networks both internally and externally
Provide support in the implementation of key projects and initiatives
Be compliant with all local site Environmental, Health and Safety regulations
Ensure compliance to all local site company policies and procedures
Ensure continuing self-development by seeking opportunities for increasing job knowledge and skills
Assist with the effective induction of new staff
Provide support with recruitment activities as required
Engage in any new requirements, following training as appropriate
Carry out any other duties as required to support the team
Assist with administration of medication, this includes depot`s, medication stock and ordering
Qualifications
Registered General Nurse or Mental Health Qualification
Valid NMC Pin
Skills and Experience
Effective interpersonal skills, with the ability and confidence to generate and build effective relationships with colleagues, service users and external professionals.
Good knowledge of the most up-to-date clinical practices
Effective Crisis Resolution.
IT Literate
Experience working in Forensic Services?or registered as Forensic?Practitioner - desirable
Knowledge of Safeguarding Procedures and Reporting protocols. MCA (Mental Capacity Act) and DOL's (Deprivation of Liberty Safeguards), Pharmaceutical Guidelines and Practices
Experience of mental health issues and/or services, either as a user, carer or employee.
Minimum of?1?years' experience?of?clinical?practice post qualification?in the community, rehabilitation services or acute inpatient settings
