To ensure the well-being of all residential and day clients at Penpergwm House, by identifying and supporting their well-being and social needs as well as co-ordinating and facilitating a seamless and stimulating programme of activities

Evidencing outcomes in line with the Social Services and Well-Being (Wales) Act 2014 and the Regulation and Inspection of Social care (Wales) Act 2016 using Blyssful Software
Promoting and enabling enjoyable social contact between the clients, families and the community
Working with, and establishing, new associated partners e.g. Cross Ash Primary School/The Bryn/local community
Promoting the Welsh language and culture in daily life
Activities:
Identifying and enabling meaningful activities to be incorporated into the everyday life of the client
Planning, facilitating and managing daily activity sessions including presenting to clients in a weekly programme format.
Determine the most appropriate activities by levels of interest, uptake and suitability using surveys and feedback
Liaising with Club Coordinators to ensure that all activity sessions run smoothly.
Ensuring that all activity equipment is in full working order and reporting to relevant people if not.
Ensuring that all volunteers are supported and working with their responsible person.
To co-ordinate and help facilitate fund raising events such as fairs, coffee mornings and raffles.
Encourage inclusion in activity sessions via 1 to 1 support for Residents in their rooms or for smaller groups if necessary.
To ensure activity register is kept up to date with relevant information on attendance.
Other:
Supporting Clients with their personal care, mobility and continence needs if they are off site
Supporting Care Team if they are short staffed by being available to help serve meals or be "eyes on" while the care staff are busy.
To conduct Risk Assessments for venues and locations
To conduct dynamic risk assessment to identify hazards and mitigate risks in every activity session
Essential Requirements:
An understanding of the well-being outcomes in line with the Social Services and Well-Being (Wales) Act 2014 and the Regulation and Inspection of Social care (Wales) Act 2016 using Blyssful Software
An ability to understand expectations of the Social Services and Well-Being (Wales) Act 2014 and the Regulation and Inspection of Social care (Wales) Act 2016 as well as the CIW Key Line of Enquiry regarding Well-Being and be able to evidence how we are achieving this.
QCF Level 2 in Health & Social Care or equivalent
Registered with Social Care Wales
At least 2 years' experience working in an older person's care environment
Understanding challenges that people living with dementia experience
A keen interest in activities and opportunities that inspire older people
A confident, proactive person who has no hesitation in speaking to strangers
Creative thinker who is always on the lookout for opportunities
Strong organisational and problem-solving skills, someone who can think on their feet
Excellent Customer Service skills
Good sense of humour and a good listener
Ability to work under pressure and be flexible
Excellent Microsoft Office and computer skills
Excellent administrative skills and recording keeping
Ability to work within a budget
Clear communicator with a good command of English
Full Driving licence to be able to drive all vehicles provided by Penpergwm, including the minibus to transport clients.

An ability to speak Welsh
