Role purpose
As Wellbeing Support Officers you will deliver our day opportunities and social inclusion provisions across the organisation, some of which are carried out in close partnerships with other teams and organisations such as the local health board and other Voluntary Sector partners. Services will be provided to people of all ages, residing in the Llanelli and Carmarthenshire area, who require Mental Health and Wellbeing support services. These are to be inclusive of Veterans and Carers.
Delivery will be through a variety of one to one, group and self-help sessions in Welcome/Resource Centres (Carmarthen and Llanelli) and in a variety of community venues such as Llandeilo, St Clear's, Llandovery, Whitland, and Ammanford.
Wellbeing Support Officers will also be responsible for the monitoring of member outcomes and wellbeing, along with KPI's through the use of PHQ9, UCLA Loneliness and Isolation Scale, GAD7, SWEMWBS and WEMWBS and assist the Coordinator in producing quarterly returns and submissions.
Principal responsibilities
Your exact role will depend on the requirements of the service, but could include:
? Reviewing referrals and undertaking assessments of member needs
? Working one to one with members
? Supporting people in care planning
? Helping people engage with activities
? Supporting and facilitating group work
? Delivering structured workshops with key outcomes
? Helping people with mental health problems to develop goals and recovery plans Expectations
? To work in line with, and to support the delivery of, the vision, mission, values and goals of Llanelli Mind and to be a champion for Llanelli Mind at all times.
? Adhere to and work within all the policies of Llanelli Mind such as child protection policy, protection of vulnerable adults, confidentiality and data protection policies, and the policies contained within the employee handbook.
? To actively participate, engage with, and respond to, the Llanelli Mind supervision, external supervision and appraisal processes.
? To attend and positively contribute to Llanelli Mind and Project meetings and other meetings as required.
? To attend and positively contribute to Llanelli Mind and Mind training as required.
? To actively contribute to a positive, supportive and constructive working ethos, relationships and environment with Llanelli Mind, partner organisations and other organisations.
? To contribute to making Llanelli Mind a greener workplace
. ? To support and contribute to our overall aim of the participation people with experience of mental health problems, including within Llanelli Mind and to be committed to working alongside people with experience of mental health problems, as colleagues, (paid & unpaid) experts and campaigners.
? To be flexible, adaptable and undertake work to support the aims of the project and Llanelli Mind, including undertaking other tasks and responsibilities required by management not listed within this role profile
