Job Title: Youth Worker

Overview:

We are seeking a dedicated and empathetic Support Worker to join our team. As a Support Worker, you will be instrumental in providing essential care and assistance to individuals requiring support. Your primary focus will be on ensuring the well-being and enhancing the quality of life for those in need.

Responsibilities:

- Assist individuals with daily tasks and activities as outlined in their care plans

- Provide emotional support and companionship to promote social well-being

- Monitor and report any changes in the individual's condition or needs

- Collaborate with healthcare professionals and family members to ensure comprehensive care

- Encourage independence and empowerment through tailored support strategies

- Maintain accurate records of care provided and progress made by individuals

Qualifications:

- High school diploma or equivalent (required); additional certifications in healthcare or social work are a plus

- Proven experience in a caregiving or support role, preferably in a healthcare or community setting

- Strong communication skills and ability to interact compassionately with individuals of diverse backgrounds

- Ability to work effectively both independently and as part of a team

- Knowledge of available community resources and support services
