The Navigator plays a key role in supporting individuals within our service that have mental health needs, learning and/or physical disabilities and/or those in addiction recovery to lead fulfilling, independent lives. Working within FY Care's values of compassion, respect, and empowerment, the Navigator provides practical and emotional support to people in their own homes and in the community.
This includes assistance with daily living, medication management, health appointments, social engagement, and the development of essential life skills. The role promotes recovery, wellbeing, and social inclusion through a person-centred and strengths-based approach.
Responsibilities
Supported Daily Living
Assist individuals with day-to-day tasks such as cooking, cleaning, shopping, and maintaining a safe, comfortable home environment.
Encourage and support service users to take pride in their surroundings and develop independence in household management.
Promote healthy routines around personal care, diet, and sleep.
Medication and Health Support
Safely support the dispensing and administration of prescribed medication in accordance with FY Care policies and training.
Accompany service users to medical and therapeutic appointments, providing advocacy and emotional reassurance where needed.
Monitor wellbeing and promptly report any health or behavioural changes to the appropriate professional.
Mental Health and Recovery Support
Build trusting relationships that promote emotional safety, recovery, and hope.
Offer encouragement and practical support to help individuals follow their recovery plans and maintain stability.
Support relapse prevention by recognising triggers and working collaboratively with healthcare teams.
Respond calmly and appropriately to crises or challenging behaviour, following FY Care's safeguarding and risk management procedures.
Social Engagement and Community Involvement
Support individuals to access and participate in social, cultural, or community activities that build confidence and reduce isolation.
Encourage the rebuilding of positive relationships with family, friends, and community networks.
Facilitate attendance at support groups, community meetings, and educational or vocational opportunities.
Life Skills Development
Maintain accurate and confidential records of daily support, progress notes, and any safeguarding concerns.
Work collaboratively with colleagues, health and social care professionals, and family members to ensure consistent, holistic support.
Attend team meetings, supervision sessions, and ongoing training to maintain high standards of care.
Skills and Experience
Experience of working with people with mental health challenges and/or substance misuse issues.
Understanding of recovery principles, harm reduction, and person-centred approaches.
Strong communication and interpersonal skills with the ability to build positive, professional relationships.
Ability to work independently and as part of a team.
Awareness of safeguarding adults and confidentiality policies.
Reliable, flexible, and organised approach to work.
Desirable Qualifications
NVQ/QCF Level 2 or 3 in Health and Social Care (or equivalent).
Medication Management and First Aid training.
Experience in supported housing, community support, or care coordination.
Lived experience of recovery or supporting others through similar experiences (valued but not essential).
