Summary



Harrow East PCN is a group of three dynamic and friendly forward thinking GP practices located in a geographically contiguous area of East Harrow. We share an ethnically diverse population with a high prevalence of Diabetes, Pre-diabetes, Cardiovascular Disease, Hypertension and frailty. Our network of GP practices work in close alignment to meet the complex needs of our patient communities, and ensure they receive effective and culturally sensitive care.

The ethos of our PCN is to support GP resilience and sustainability, by creating system efficiencies from economies of scale through a centralised multi-disciplinary team who work at scale across our member practices.

The PCN Care Coordinator is an essential part of this team, helping to ensure that patients receive the appropriate care while reducing duplication of work at individual practice level. The PCN Care Coordinator will work across our member practices to coordinate patient care, support initiatives that help to help tackle health inequalities and educate and empower patients to manage their care with a view to increasing GP capacity, so that our practices have more time to focus on patients with more reactive and complex needs.

Job responsibilities



The main job responsibilities include but are not limited to the following:

Supporting the delivery of local enhanced services that are designed to increase local population health (for example Diabetes, Mental Health, Hypertension). Supporting delivery of standardized GP checks (for example immunizations, cancer screening, learning disability, health checks). Triaging and processing online consultation requests for patients who contact their GP surgery online, through effective care navigation. Developing and using culturally sensitive methods of communication to engage with our hard-to-reach communities, to help reduce health inequalities. Supporting new PCN initiatives which help to enhance the resilience and sustainability of our member practices. Using a range of IT systems including the Microsoft package, EMIS Web, AccuRx.

In undertaking duties outlined in this job description the post holder will have access to confidential/sensitive information relating to local patients registered at our member practices, other healthcare workers/colleagues & local businesses.

Such information is strictly confidential and as such the post holder will be expected to act in line with the company's confidentiality policies and procedures to ensure protection of personal and confidential data.

The post holder will be supported with personal/professional development by the PCN manager, and will be expected to take responsibility for their own development and learning in a collaborative approach.

Criteria



Education & Qualifications



Educated to GCSE/O Level standard or equivalent, including English Language and Maths IT literate

Skills & Experience



Evidenced experience of high level administration Excellent organisational and communication skills Experience of supporting administration of local NHS services Ability to work to deadlines and prioritise workload accordingly Experience of Microsoft Office packages (to include Word, Excel, outlook, MS Teams and Powerpoint) Experience of document management Experience of using Pinnacle Experience of using EMIS Able to work methodically on process driven tasks
