Bridge House is a new 4-bed transitional home for young adults, aged 18-25, with a learning disability and autism offering 3 year residential placements to develop life skills and confidence.

Job Summary
As a Support Worker at Bridge House, you will play a vital role in supporting the young adults to overcome barriers, build resilience, and achieve their personal goals. Working closely with our small team and the young adults we support, you will help create a safe, structured, and empowering environment that fosters independence and well-being.
Evening and every other weekend working.
Key Responsibilities
Provide person-centred support
with personal care, daily living activities, and meal preparation, promoting independence, dignity, and healthy lifestyle choices.
Build positive, trusting relationships
with young adults, acting as a consistent role model and providing emotional support that enhances confidence, self-esteem, and wellbeing.
Implement individualised support and behaviour plans
, using positive behaviour strategies tailored to each young person's needs, goals, and developmental stage.
Facilitate one-to-one and group activities
that develop social skills, emotional regulation, and life skills in a supportive and inclusive environment.
Maintain accurate care records and reporting
, monitoring wellbeing and promptly communicating any changes in health, behaviour, or needs to the appropriate professionals.
Work collaboratively within the care team
, upholding safeguarding policies and contributing to a safe, homely environment that reflects best practice and our organisational values honest, caring, respectful, motivated and ambitious.
Skills
Previous experience in a support worker / caring role is preferred but not essential.
Strong interpersonal skills and the ability to build rapport with young adults.
Ability to manage behaviours that challenge effectively and compassionately.
Excellent communication skills, both verbal and written.
A caring attitude and a genuine desire to help others.
Initiative to plan activities and engage the young adults in new and innovative ways.
