An already established Ofsted Registered Support Company in Southampton has a rare employment opportunity for a Support Worker Position working with 16+ Young People in Care and Care Leavers in Southampton. Phoenix House is a 9 bed house close to the city centre staffed 24/7. The support worker role will work a varied shift pattern (10 hours) across 5/7 days; working every other weekend.

Whether you're new to supporting people and fancy a career change, or have years of experience in social care, you will receive the training and support you need to do your job really well.

We are looking for passionate, optimistic, professionally curious and open-minded people to join our eclectic team of dynamic members, all striving for the same person-centred goal of supporting Young People towards independence.

To be part of a team offering a comprehensive support service to young people using a PACE approach and trauma informed practise. The aim being to ensure the physical, social and emotional wellbeing, development of life skills to build confidence and resilience through daily structure and routine; with the overall all objective of moving on positively into independent accommodation.

Phoenix Independence offers a caring, supportive semi-independent accommodation for the young to build life skills to move on into independence. We have a number of years' experience in high quality support provision for 16-24-year-olds, with our focus centred on the needs the individual.

The welfare of our young people is paramount in the role of a Support Worker. Not only do they help keep young people safe but contribute to young people making informed decisions and positive life choices and help develop skills for their future.

KEY ACCOUNTABILITIES

Work according to and adhere to Phoenix Independence's Policies, Procedures and Values. Respond to the needs of the project by undertaking other tasks which may be requested by the Manager on occasions, which may not be included in the specific job description noted above, but which reflects the general nature and level of the role. Act as a role model in all you do and represent the business professionally and positively in accordance with our behaviours and values Put the Young Person and their safety at the heart of all that you do Work with colleagues and managers to deliver against our Business Plans Familiarise yourself with and work within all company policies and procedures Actively engage in learning opportunities; identify better and more efficient ways of doing things Be familiar with and fully compliant with all localised safeguarding requirements and internal Safeguarding Policy and protocols Able to engage with young people and build relationships in a positive and proactive manner Self-motivated with a positive attitude Able to work a shift rota which includes evenings and weekends
Developing Trusting Relationships

Be a positive role model, nurture and show support for all young people Adhere to the Code of Conduct / Professional Boundaries / Safeguarding and Lone Worker policies at all times Utilise skills developed as a result of on-going reflective practice to continually improve effectiveness in order to enable residents to achieve their outcomes and move on to independent living. Be an allocated key-worker for specific young people Promote health and development to young people as appropriate to their needs Maintain a positive and informed response to young people and their behaviour Provide and participate in leisure activities with young people in line with their care plans. Support young people with education in a non-teaching capacity
Co-ordination of Support and Multi-Agency Working:

Take responsibility for key working a caseload. Use listening and communication skills to develop positive relationships to enable young people to; build and develop daily living skills, confidence and resilience to reach the overall goal of independent living Mentor and coach customers so that they can reach their individual goals and aspirations Support customers to develop multi-agency support and risk plans which enable them to move towards independent living and education/ work/ training and volunteering opportunities Work closely with any relevant agencies in relation to young peoples support and risk assessments /management plans, attending multi agency meetings where appropriate. Use psychologically informed skills to develop relationships which enable customers to feel confident regarding the holding and sharing of personal data. Adhere at all times to the data protection and confidentiality policies
Safeguarding and Customer Well-being:

Take an active interest and participate in ensuring all young peoples well-being is maintained. Follow all procedures in relation to responding to and recording serious incidents or accidents (including safeguarding) Report and record all Safeguarding concerns to the designated Safeguarding Officer and follow local reporting procedures and any advice and guidance given from the Local Authority and Safeguarding Lead
Early Intervention Guidance:

Provide advice and guidance to residents where required across all areas of the Outcomes Star - develop SMART goals and objectives that are achievable for the individual Signpost and coordinate access to specific advice and guidance for all young people where this is identified as a requirement Use positive risk taking to enable the customer to move forward and make positive informed decisions
Co-ordination of Move-on and back to Education/Training/ Work Opportunities:

Promote and coordinate learning opportunities within the local community Enable young people to engage in learning opportunities which maximise their opportunities to move on to more independent living
Enabling Social and Community Inclusion:

Co-ordinate and/or take part in social and leisure activities with residents which promote social and community inclusion and support back to work and independent living skills Ensure there are a range of options for customers to be involved with and/or influence the delivery of services.
Enabling Social and Communal Living:

Support customers to develop skills which will support their ability to move-on from the service and live independently.
Complaints and Compliments Management:

Take ownership and responsibility, responding to young people in order to identify issues prior to them escalating to formal complaints where possible. Adhere at all times to the Complaints policy within set timescales 100% of the time.
Tenancy Management and Health and Safety:

Promote a positive culture within the home and a homely environment. Maintain structure, boundaries and domestic routines within the home. Be responsible for the Health and Safety of the home, young people and yourself Complete and maintain administrative records as required by the home. Work to ensure that residents are able to maintain their tenancies. Specifically works with young people to maximise income in order to make regular rent payments where appropriate. Identify areas for development for young people to increase skills in order that they can move-on to independent living. Support residents to avoid exclusions, evictions and abandonments.
Staff and Resident training:

Take a proactive role in recognising and developing personal training needs. Provide opportunities for young people to engage in learning opportunities both within the service and externally.
Continued Personal and Professional Development:

Take ownership of own personal and professional development Attend and participate in supervision and staff meetings as required Undertake relevant mandatory training in Essential, Core and Advanced subjects. Act as a role model in all you do and represent the business professionally and positively in accordance with our behaviours and values Put the customer at the heart of all that you do Work with colleagues and managers to deliver against our Business Plans Familiarise yourself with and work within all policies and procedures Actively engage in learning opportunities; identify better and more efficient ways of doing things Be familiar with and fully compliant with all localised safeguarding requirements and Phoenix Independence Safeguarding Policy and Protocols.

PERFORMANCE MANAGEMENT:

There are various targets you will be expected to meet as part of the role. These are measured against the service and on an individual basis. We encourage our team to inspire change in every interaction you have whether with customers, staff or external agencies. We expect you to proactively help the service to achieve its Key Performance Indicators (KPIs)
